Right-Sided Pain in Early Pregnancy: A Guide
What Causes Right-Sided Pain in Early Pregnancy?
Right-sided pain in early pregnancy is usually nothing to worry about, but it does need the same careful assessment as left-sided pain. Your right side is home to your right ovary, the right round ligament, part of your bowel, and your appendix — so there are several possible sources of discomfort.
Most right-sided aches in early pregnancy are due to normal stretching and hormonal changes. But because ectopic pregnancy can occur on either side, red-flag symptoms must always be taken seriously.
Common Benign Causes
- Round ligament pain — a sharp, brief stretch on the right side as your uterus grows and the supporting ligament lengthens.
- Corpus luteum cyst — the cyst on your right ovary that sustains early pregnancy can produce a dull ache.
- Constipation and wind — hormones slow your digestion, causing right-sided bowel discomfort.
- Implantation cramps — mild, brief cramping as the embryo embeds can feel one-sided.
- UTI — right-sided pelvic ache with burning on urination needs a urine sample check.
Rarer Causes
- Appendicitis — pain starting around the belly button and moving to the lower right, often with nausea, fever, and loss of appetite. Needs A&E.
- Ovarian torsion — sudden severe pain if an ovary twists on its stalk. Needs urgent surgery.
When Right-Sided Pain Is a Red Flag
Contact your EPU, GP, or 111 urgently — or 999 if severe — if:
- Pain is sharp, severe, or worsening
- You have shoulder-tip pain (internal bleeding sign)
- You feel dizzy, faint, or collapse
- There is bleeding alongside the pain
- You have fever or vomiting
- Pain does not ease with rest
An ectopic pregnancy on the right side is possible and is a medical emergency requiring prompt treatment.
What Happens at the EPU
You may be offered:
- A transvaginal ultrasound to confirm the pregnancy location
- Blood tests for hCG levels
- A urine test to rule out infection
- A pelvic exam
If an ectopic is confirmed, treatment may involve medication or surgery. Early detection is vital.
Key Takeaways
Right-sided pain is usually round ligament stretching, a cyst, or digestion. But ectopic pregnancy and other serious causes can occur on the right side too. Report severe, worsening, or bleeding-associated pain urgently — never delay when red flags appear.
Reassurance for Benign Pain
If your scan confirms the pregnancy is in the uterus and your pain is from stretching or a cyst, you can be reassured it is a normal part of early pregnancy. These aches usually ease over time as your body adjusts.
